Declaration

I, the undersigned, hereby declare my wish to use the facilities located at 2 Osvobozhdenie Square, Shumen (hereinafter the “venue”), and to participate in the activities at the venue provided and organised by “Di Key 15” OOD, UIC 208024304, with its registered office and management address at 4 Pliska Street, floor 1, apartment 27, Shumen, Shumen Municipality (hereinafter the “company”). By this declaration, I confirm, undertake and agree on my own behalf/on behalf of my child that:

1. I have read and fully understood Safety Rules at the venue (I have explained to my child in an understandable way Safety Rules   at the venue) and agree to comply with them (and ensure my child complies with them) when participating in the Activities.
2. I confirm that I (my child) will comply with all additional instructions provided by the company and/or its staff at all times before and during the Activities.
3. I understand that my participation (my child's participation) in the Activities involves known and unknown risks that may result in physical or emotional injury, paralysis, death, or harm to participants, third parties or property. I understand that although certain rules, equipment and personal discipline may reduce this risk, the risk of death or serious injury remains.
4. I confirm that I (my child) have no illnesses or injuries, that I (my child, where applicable) am not pregnant, and that I (my child) have no condition that could affect safe participation in the Activities or increase the risk of injury, including injury to others. I confirm that I (my child) am in sufficiently good physical condition to participate in the Activities.
5. I confirm and agree that I am responsible for my actions (my child's actions) in accordance with   Safety Rules   at the venue, to ensure the greatest possible safety for me (my child) and the people around me (my child) while participating in the Activities. I confirm and accept that the Activities may take place without direct supervision by the company and/or its staff. If I notice any unusual and/or significant hazards during my participation (my child's participation), I will immediately stop my participation (my child's participation) and bring those hazards to the attention of the nearest company employee.
6. I understand and agree that the Activities provided by the company require a reasonable level of physical fitness and skill. I confirm that I (my child) will undertake only Activities appropriate to my (my child's) skill level and which I (my child) can control at all times.
7. I confirm that I fully understand the risks, both known and unknown, of participating in the Activities. I freely assume and accept all such risks. I assume full obligation, responsibility and risk for my participation (my child's participation), except to the extent that the company is not legally permitted to exclude such liability. I further confirm that my participation (my child's participation) in the Activities is voluntary.
8. I agree that the company and/or its staff may provide first aid and/or other life-saving medical treatment if I (my child) suffer injury or illness. I agree to release the company and its staff from any liability or claims resulting from such treatment to the fullest extent permitted by law.
9. I confirm that the company is not and will not be liable for any injuries, damage and/or loss to persons or property suffered by me (my child) in connection with and/or arising from my participation (my child's participation) in the Activities, including incidents resulting from my failure (my child's failure) to comply with Safety Rules at the venue, unless such injury, damage and/or loss results from wilful misconduct or gross negligence by the company or its employee and/or representative.
10. I agree to indemnify the company in full for any loss, liability, expense, claim and/or damage it suffers arising from my participation (my child's participation) in the Activities.
11. I confirm and agree that photographs and videos of relatives and/or friends taken by me (my child), with the company's permission, during participation in the Activities will be used solely for private, non-commercial purposes, and for no other purposes, and will not be published online without the express consent of the people shown, given in a manner that can be evidenced.

1. I am aware of and familiar with all measures to reduce the spread of infectious diseases at the venue, which I have explained to my child clearly and understandably.
2. My child and I will follow the rules, instructions and measures to prevent the spread of infectious diseases during our stay at the venue, which are clearly displayed at the venue and on the company's website.
3. I understand and accept that during our stay at the company's facilities there is a risk of contracting an infection or infectious disease and/or transmitting infectious diseases.

CONSENT TO PERSONAL DATA PROCESSING

1. By signing this Declaration, I consent to the company processing and storing my personal data/my child's personal data (first name, last name, date of birth, email and mobile phone number) to administer reservations and registration for participation in the Activities, including sending documents and additional information relating to my participation (my child's participation) and exercising other rights and obligations connected with participation in the Activities, for 3 years from the date of signing. I understand that I may withdraw my consent at any time by sending the company a written notice in free form, request information about data processing within reasonable limits, read more about data processing and my rights in the company's Personal Data Processing and Protection Rules, and seek assistance from the company by email: gdpr@gravitypark.bg
2. By signing this declaration, I consent to being recorded by the automatic surveillance cameras installed at the company's venue and to the recorded personal data being used to protect the health of visitors and staff and the property of visitors and the company. I am informed that personal data collected through video surveillance will be processed for no longer than 6 months after recording.

I have read this Informed Consent Declaration, fully understand its terms, and accept them freely and voluntarily, without any coercion towards me (my child). This Declaration is valid for 3 years from the date of signing and applies to all visits during that period. Before the signed declaration expires, I agree that the company may contact me to sign a new declaration. If the declared circumstances change while the Declaration is in force, including but not limited to changes in my health or my child's health, I undertake to notify the company immediately before my participation (my child's participation) in the Activities and to sign a new Declaration. Consent to photographs/video: I agree that I (my child) may be photographed and recorded while participating in the Activities and that the resulting photographs/videos (“Recordings”) may be used for advertising and/or marketing purposes, particularly publication on the company's website. I confirm that this consent grants the company unrestricted copyright rights over the Recordings and the ability to use, modify, reproduce or broadcast the photographs as it considers appropriate, without compensation to me or further approval for such use, in accordance with applicable data protection and other legislation. I understand that I may withdraw my consent at any time, request information about data processing, and seek assistance from the company by email: gdpr@gravitypark.bg, without losing my right to participate (my child's right to participate) in the Activities.
